Online Registration Form

Please use this form to register online for your course. You will have an opportunity to choose your clinical hours and register for CPR if desired on the first day of class. You may also register by phone at 801.759.5164.

You must send a check or money order in the amount of $75.00 to hold your place in a CNA class. You must send a check or money order in the amount of $100.00 for the Basic or Advanced Phlebotomy Course. The balance due for each class is due on the first day of class.

PLEASE do not register for classes lightly. This means you should only register if you fully intend to attend the course -- check dates on the calendar page carefully. Please do not register if you do not KNOW you will attend the class for which you registered. However, you are not officially registered until we receive your deposit. The balance is due the first day of class.

We DO NOT ACCEPT CREDIT CARDS. Please bring cash, check or money order only. Thanks.
